Supportive Housing

Supportive Housing is a housing option that provides personal support services and homemaking in permanent community based congregate residential settings. The target population is primarily frail and/or cognitively impaired older adults who can no longer manage in their own home with available resources but are not yet ready for personal care home (PCH) placement. Individuals require support through a service package that includes meals, weekly housekeeping and laundry. Staff provide episodic support and cueing for activities of daily living such as bathing, dressing, and medication reminders. For more information on the Long-Term Care Panel Process please contact your local Home Care office and speak to a Home Care Case Coordinator.
