If you are not the patient / client / resident:

  • The Personal Health Information Act (PHIA) states that consent must be obtained from the patient/client/resident to discuss their health information with anyone other than the patient/client/resident.
  • Here is a link to the Patient Advocate Agreement Form on the Manitoba Institute for Patient Safety website. Kindly have the individual for whom you are advocating complete this form, designating you as their advocate. We will request this form when we reach out to discuss your concern.
